After my divorce, the practical disasters came fast — no home, no job, a custody case that dragged on long after the rest was fixed. I solved the housing. I solved the job. The court case just sat there, month after month, a low hum of stress I couldn't close the file on. Eventually my body answered for me: I couldn't keep food down. In one primary-care study of first-episode depression, nearly 44% of referrals to a specialist carried no documented clinical justification at all. Not wrong, necessarily. Just not reasoned through — closer to a coin flip than a diagnosis.
None of this means antidepressants or painkillers are illegitimate. It means the fastest tool in the room gets reached for before the slower, better one — a diagnosis, a referral, a lifestyle intervention — gets a fair look. What follows is what the actual published evidence says about both drug classes, and what a terrain-level alternative looks like for the two problems they're most often used to solve.
What the Antidepressant Data Actually Shows
A widely shared Mad in America article this month claimed antidepressants can "change peaceful citizens into killers," built on a real 2015 Finnish study of 959 convicted murderers (Tiihonen et al., World Psychiatry) that found a 31% higher homicide risk associated with antidepressant prescriptions. That number is real. What the article didn't mention is what else was in the same study.
The Study's Own Comparison Group Tells a Different Story
The same Finnish data found benzodiazepines at +45%, opioid painkillers at +92%, and ordinary anti-inflammatory painkillers — the same NSAIDs in most bathroom cabinets — at +206%, nearly seven times the antidepressant figure. A separate case-control analysis run in the same paper found the antidepressant result wasn't statistically significant at all; only the simpler cohort comparison was. Antipsychotics, given to the most severely ill psychiatric patients in the study, showed no elevated risk — the opposite of what "sicker patients get more dangerous drugs" would predict.
If there's a signal here worth taking seriously, the data points hardest at a drug nobody needs a prescription for, not at psychiatry.
Two large Swedish registry studies since (Molero 2015, Lagerberg 2020 — over 850,000 patients combined) looked for the same antidepressant-violence link. Where they found anything, it was almost entirely confined to patients under 25, with no clear dose-response relationship, and the authors' own conclusion was blunt: the association "cannot be interpreted causally" — the depression itself, not the pill, is the more likely driver.
The one case where a jury did formally find a pharmaceutical company responsible is worth knowing, because it isn't the case most often cited.
The $20 Million Secret Nobody Talks About
In 1989, Joseph Wesbecker shot 8 people dead at a Louisville printing plant a month after starting fluoxetine (Prozac). The 1994 trial against Eli Lilly ended in the company's favor — but the presiding judge later discovered Lilly had secretly paid the victims' families $20 million just before the verdict, without telling the court. He called the trial a "sham" and had the official record changed from "verdict for defendant" to "settled." That's a stronger indictment of institutional conduct than the shooting itself — and it's the detail most retellings leave out.
The part of the antidepressant story that gets the least attention isn't violence — it's what happens when someone tries to stop.
NICE Had to Correct Its Own Guidance
Until 2019, the UK's official prescribing guidance described antidepressant withdrawal as "usually mild and self-limiting over about one week." A 2019 systematic review of 14 studies found the real average: 56% of people who stop or reduce an antidepressant get withdrawal symptoms, and 46% of those call them severe — sometimes lasting months. NICE formally amended its own guidance afterward to acknowledge "symptoms lasting much longer... and being more severe for some patients." That's not a fringe critique. That's a national guideline body correcting itself on the public record.
The Painkiller Nobody Warns You About
Ordinary NSAIDs — ibuprofen, diclofenac, naproxen — carry the largest single risk signal in the Finnish homicide study above, at three times the antidepressant figure. The proposed mechanism is real pharmacology, not speculation: prostaglandins normally suppress dopamine release, and blocking that suppression can trigger a dopamine surge in sensitive individuals — documented, in rare cases, as paranoid psychosis. There's even a named clinical category for it: NSAID-induced Othello's syndrome, delusional jealousy severe enough to be genuinely dangerous. The strongest suspects aren't ordinary ibuprofen but indomethacin and selective COX-2 inhibitors specifically — chemically closer to serotonin in structure than the more common NSAID classes, which likely explains why the effect doesn't generalize evenly across the drug class.
That's the rare, dramatic risk. The common, quiet one is older and much larger in scale.
15,000–20,000 Deaths a Year, From an Aisle-Six Drug
Long-term NSAID use is linked to an estimated 15,000–20,000 deaths and over 100,000 hospitalizations a year in the US from stomach bleeding and ulcers — a decades-old but still-repeated estimate, no fresher national recount available. For comparison: this is a drug bought without a prescription, taken casually, for headaches and sore backs. And unlike SSRIs, no NSAID carries anything close to a black-box warning for this. A 2025 pharmacovigilance study of nearly 59,000 ibuprofen adverse-event reports found real psychiatric-symptom signals too — higher than ketoprofen, aspirin, or diclofenac specifically for ibuprofen. There is also, as far as could be found, no legal case against an NSAID manufacturer parallel to the SSRI lawsuits above — likely because these are generic, over-the-counter drugs with no single deep-pocketed defendant, not because the risk is smaller.
What's Actually Driving This
The "low serotonin causes depression" story that most people were taught is not what the evidence supports. A 2022 umbrella review found no consistent evidence for it across six major lines of research — contested by other psychiatrists on methodology, but consistent with what specialists had already suspected for years: SSRIs can work without the original theory being correct. The more defensible picture that's replaced it isn't simpler, but it is more useful, because it points to things you can actually address.
The Terrain Response

For Pain — the Willow/Turmeric Route
Willow bark works through the same salicylate family as aspirin, but the whole plant — buffered release over roughly two hours instead of a sharp peak, plus protective tannins — has a gentler profile than isolated aspirin, and no tradition across four independent millennia-old lineages ever recorded a psychiatric side effect from it. Turmeric adds a second inflammatory pathway (5-LOX, not just COX) that most pharmaceutical NSAIDs miss entirely. For Mood — Layered, Not a Single Pill
Lavender's oral extract (Silexan) matched lorazepam head-to-head in a clinical trial — a real benzodiazepine-grade result. Chamomile produced a 60% drop in anxiety symptoms in a 12-week trial. Ashwagandha is the most direct cortisol lever available botanically, cutting morning cortisol by 66% against a 2% placebo effect in a controlled trial — the single strongest hormonal intervention in this entire piece. For genuine major depression, St. John's Wort matched SSRIs head-to-head in mild-to-moderate cases (though not in more severe major depression), and saffron matched fluoxetine and imipramine directly in small but consistent trials.
None of this is a substitute for medical care, especially for severe depression or any suicidal ideation — that needs a doctor, not a tea. St. John's Wort has real drug interactions: it weakens the contraceptive pill and warfarin, among others. 5-HTP should never be combined with an SSRI or MAOI — the serotonin-syndrome risk is real. Kava has genuine anxiolytic strength but a documented liver-safety history tied specifically to using the wrong plant part (leaf/stem instead of root) — root-only, water-extracted preparations only. If you're already on a prescribed antidepressant, don't stop it abruptly on the strength of an article; the withdrawal data above is exactly why that matters.
The thread connecting all of it back to my own story is the vagus nerve — the same route breathing exercises use. Fiber intake feeds it via short-chain fatty acids; hop bitter acids activate it directly; and it's the reason a non-pharmacological intervention was able to do, for me, what a prescription pad was the only thing offered.
