Restore the exhausted immune terrain after viral loading. T-cell exhaustion and NK-cell suppression are the hallmark post-viral immune dysfunction — not too little immune response but a wrongly calibrated one. Astragalus membranaceus is the only botanical in a direct Long COVID Phase 2 RCT (MYPplus, PMC12034194) with significant reduction in severe fatigue. Nigella Sativa reduced COVID-19 mortality in meta-analysis of 7 RCTs (PMID 38455176).
Restore the gut-brain axis that is structurally disrupted post-virally. Liu 2022 (PMID 35082169, Gut) documented persistent gut dysbiosis in Long COVID: F. prausnitzii and Bifidobacterium pseudocatenulatum show the strongest inverse correlation with symptom burden. The only post-COVID RCT with clinical evidence: SIM01 synbiotic (Bifidobacterium spp. + prebiotics) produced 2.3–2.6× greater improvement in fatigue, brain fog and GI symptoms vs placebo at n=463 (PMID 38071990, Lancet Infect Dis 2024).
Restore mitochondrial ATP production — the cellular energy crisis underlying post-viral fatigue. Cordyceps militaris increased VO2max +4.8 ml/kg/min and time to exhaustion in RCT (PMID 27408987). Mechanism: adenosine analogues stimulate ATP synthesis via AMPK/mTOR pathway and increase antioxidant enzyme activity. PQQ stimulates mitochondrial biogenesis — critical after viral mitochondrial damage.
Address post-viral brain fog and neuroinflammation. Lion's Mane (Hericium erinaceus) induces NGF synthesis in astrocytoma cells via JNK signaling (PMID 18758067) — the mechanistic basis for cognitive recovery after neuroinflammation. Reishi increased CD3+, CD4+, CD8+ T-lymphocytes and improved CD4/CD8 ratio in human RCT (PMC9914031). No direct Long COVID neurocognitive RCT exists for these herbs — mechanism-driven use.
Stabilize the HPA axis and prevent relapse through chronic cortisol suppression of the immune system. Post-viral patients are caught in a negative spiral: viral damage → HPA dysregulation → immune suppression → slower recovery. Ashwagandha breaks this cycle via −66% cortisol reduction (RCT 2022). Andrographis paniculata showed clinical recovery benefit as add-on in mild-moderate COVID (PMID 36570418) — also deployable as post-phase adaptogen.
